1. Private Lesson Policy for adults
1.1 Rescheduling and Cancellation Policy for Private Lessons
- Each student can request to reschedule their scheduled time frame (once per month) at least 48 hours prior the start of the lesson.
- Cancellation due to illness will be excused only if Tennis by Dennis s is notified 24 hours in advance.
- If a student does not show up to the lesson (without rescheduling or emergency/illness situation), they are still responsible for full payment of the lesson.
- In the event of a rain, players are not responsible to attend a lesson, but are requested to check either the website or their preferred form or communication (txt or email) to see what is the status of a court and if the lesson is cancelled.
2. Group Lesson Policy for adults
Group lessons can be purchased either as a package or can be attended on a drop in basis.
2.1 Cancellation policy for group lessons
- In the event of a rain, there will be no group lessons, but the students are requested to check either the website or their preferred form of communication (txt or email) to check on the condition of a court and to make sure lesson is really cancelled. They will receive credit for the rained out class.
- If a student purchases a package of classes, they are allowed to reschedule once per month, due to sickness or other emergency at least 24hours before the class. Make-ups have to be completed in the same month, or same semester if that is applicable.
3. Waiver of Liability for adults
I agree to hold Tennis by Dennis (as well as any employees, partners, supervisors and volunteers), harmless from any liability, loss, or personal expenses arising out of private or group tennis lessons. I assume all risk and hazards incidental to my participation in the activities of Tennis by Dennis. In consideration of Tennis by Dennis I agree that while using any of the property, facilities, parking lots, buildings, fields, equipment and services in the event there is either personal injury, illness or death or damage to the personal property, personal items, car or money I will not sue or bring any action against Tennis by Dennis.
I also give my permission for the use of any photos or videos taken at tennis lessons for publishing purposes.
4. Medical Information & Background & Policy for adults
In the event of medical emergency, I give Tennis by Dennis and her staff the authority to act on my behalf if any medical care is needed. This includes, but I not limited to, first aid, injury prevention, emergency transportation, medical administration and/or any on-site care. I do hereby give my consent to medical care, emergency or otherwise, inclusive of any necessary transportation, in the event of the injury. I agree to bear the expense of any such treatment, not covered by my insurance listed below.
I also state and agree that I am in a good medical condition at the time of enrollment.

I also state and agree that the child (he/she) is at the time of enrollment in a good medical condition.
